Here you will find the enrollment forms for City Charter High School.

*2010-2011 Enrollment Form 

The 9th grade roster for 2010-2011 is filled. If you wish to sign up for the waiting list please fill out the application and attach below listed documents

 *Please make sure you submit all the documents lsited below with your enrollment form in order to complete the enrollment process for your child.

•  First proof of residency (Driver’s license, State ID, Voter Registration Card)  
• Second proof of residency (Utility Bill, Mortgage Deed/Lease)  
• A copy of your child’s birth certificate 
• Legal guardianship documents (If you are not the child’s biological parents)  

 Change of Address

Please be advised that if your child is already enrolled at City Charter High school and you have moved to a new address you need to fill out a Change of address form and include 2 new proofs of residency of the new address.
